Assessment is an essential component of the student teaching/internship semesters for our pre-service teachers. Use of assessment tools aligned with Kansas education standards help our students better understand expectations and set goals for their performance. Below we have outlined the evaluation guidelines for our clinical and university supervisors based on the content area of your student teacher and/or intern. For access to the actual forms or to complete a form online, click on the link.
You are required to complete one formative assessment form half way through the experience. This is a hard-copy form that you receive from your student’s university supervisor, the Field Placement Office or you can print out a copy using the link above. Please discuss the ratings with your student teacher and then submit it to your student teacher’s university supervisor when it is completed. You are also responsible for completing the summative assessment during the last week of the experience. This form is completed online. You will receive an email from our Field Placement Office during the semester with your username and password. You will use this to log into our system and complete the evaluation.
